The Direct Care Supervisor at Pathfinder, Inc. is responsible for overseeing the initiation and completion of procedural steps necessary for the establishment and implementation of supported living services for individuals assigned to Medicaid Waiver services. This role involves coordinating and monitoring an individual's service plan, ensuring that services are delivered effectively, and tracking progress. The supervisor is also responsible for hiring, training, supervising, and evaluating Supported Living staff, ensuring that they adhere to the required standards and regulations. In this position, the supervisor conducts preliminary assessments for new referrals and assists in developing service plans. They participate in continued stay reviews and personal future plans, determining staffing needs for direct care service delivery. The supervisor arranges for the delivery of services by conducting interviews, hiring staff, managing payroll, and overseeing the evaluation and discipline of Supported Living staff. They also participate in team meetings with family members, professionals, and care coordinators to implement procedures that ensure compliance with regulations. The Direct Care Supervisor coordinates schedules for both waiver and generic services, ensuring that all alternative living services are adequately staffed. They perform Medicaid billing payroll functions for Supported Living staff weekly, ensuring the integrity of billing processes by approving time and attendance records, reviewing direct care travel, and documenting work performed. Regular monitoring of in-home services is essential, including ensuring compliance with progress notes, required paperwork, and agency requirements for staff employment. Additionally, the supervisor is responsible for training new Supported Living staff, reviewing their progress notes, and inputting data into consumer workbooks. They create quarterly reports focusing on recipients' progress in meeting their goals and objectives. The supervisor also counsels recipients and families regarding staffing issues and is on call to assist with staffing emergencies. Monitoring client medication regimens is a critical aspect of the role, ensuring that staff are aware of medications being used and that screenings are performed as required. The supervisor prepares necessary paperwork for new plans and continued stay reviews, advocating for individuals to access a wide range of services. They document services needed but not available in the local community and work towards developing these services. Maintaining the personal rights of individuals, including provider choice and legal rights, is a fundamental responsibility of the Direct Care Supervisor.
