As a Direct Support Professional at Source of Hope, you will provide high-quality, compassionate care to individuals, helping them live safely and independently in their homes and stay engaged in their communities. This role is for dependable, compassionate individuals who are patient, responsible, and dedicated to making a real difference in someone's daily life.

Requirements

Reliability is critical-clients depend on you.
Advance notice (minimum two weeks) is required for time off
Willingness to learn, ask questions, and follow care plans carefully
Professional, respectful communication with clients and families
A strong sense of responsibility and integrity
At least 3 months of hands-on caregiving or related experience
Valid driver's license and reliable transportation (if applicable)
Ability to pass required background checks

Responsibilities

Assist with daily living activities such as bathing, dressing, grooming, and meal preparation
Support mobility needs (including wheelchair transfers when needed)
Provide transportation to appointments, errands, and community activities
Help maintain a clean, safe, and comfortable home environment
Offer reminders for medications and daily routines (per care plan)
Observe and report any changes in health or well-being
Communicate updates with families and care teams
Follow each client's Individual Support Plan (ISP)
