About The Position

Reporting to the Manager of Nutrition, you will work as an active member of our interdisciplinary team. You will be responsible for providing quality nutrition care and education to outpatients with Diabetes. In addition, you will be required to contribute to program planning, implementation and evaluation in an interdisciplinary team concept. You will practice according to the current Standards of Practice of the College of Dietitians of Ontario, and the Policies and Procedures of the Nutrition Department and the Scarborough Health Network.

Requirements

  • Member of the College of Dietitians of Ontario (CDO)
  • Certified Diabetes Educator (CDE)
  • 1-2 years recent experience as a Registered Dietitian with experience in Diabetes
  • Demonstrated knowledge of clinical/nutrition guidelines appropriate for those with Diabetes
  • Demonstrated ability to plan programs and develop educational materials to support nutrition education
  • Demonstrated exceptional interpersonal, verbal and written communication skills and judgement when relating to patients, families, co- workers and members of the interdisciplinary team
  • Demonstrated ability to work effectively within an interprofessional team and independently
  • Demonstrated ability to build and maintain effective relationships with a diverse range of people
  • Demonstrated ability to organize workload and establish priorities effectively
  • Demonstrated problem solving skills and ability to exercise independent judgement and to assess situations and problems efficiently and effectively
  • Motivated self-directed learner, who takes initiative and seeks learning opportunities to enhance patient care and professional competence.
  • Demonstrated commitment to the philosophy of patient centered care

Responsibilities

  • Providing a specialty-based focus, functioning in a dietitian role demonstrating a high level of autonomy and expert skill to formulate clinical decisions and appropriately care for outpatients with Diabetes
  • Conducting screenings and client assessments to identify the need for nutrition interventions
  • Providing education and counseling for patients and their families
  • Acting as a resource to members of the inter-professional health care team
  • Completing required documentation, including client charting, workload statistics, and quality assurance activities
  • Attending client care rounds and educational sessions
  • Participating in the design, implementation and evaluation of continuous quality initiatives
