About The Position
Assure residents balanced nutrition needs due to specific medical conditions with food preferences to achieve agreed upon goals.
Requirements
- Bachelor’s Degree in Food and Nutrition or Dietetics required
- ADA registered or registration eligible required
- Ability to interact with elders and care for their needs
- Ability to lead and coordinate team activities
- Interpersonal skills: support emotional, physical and social well-being of elders and Core Team
- Problem solving / decision making
- Technical expertise in Matrix Care, MS Office, and BluePrint
- Walking, standing for lengthy periods of time.
Nice To Haves
- Certified Dietitian/Nutritionist in NYS preferred
- 1 year in long term care preferred
Responsibilities
- Responsible for nutritional assessments and monitoring of assigned residents
- Collaborate with clinical teams to facilitate appropriate nutritional care and care planning on an as needed basis
- Provide diet counseling to residents and their family members as needed
- Act as a liaison for policy and procedures pertaining to clinical nutrition
- Serve as a liaison between foodservice and clinical nutrition needs; including menu updates, product updates and changes and coordination of therapeutic and mechanically altered dietary needs
- Help with other duties as needed that support the Core Team to fulfill St John’s mission vision and values
