About The Position

Assure residents balanced nutrition needs due to specific medical conditions with food preferences to achieve agreed upon goals.

Requirements

  • Bachelor’s Degree in Food and Nutrition or Dietetics required
  • ADA registered or registration eligible required
  • Ability to interact with elders and care for their needs
  • Ability to lead and coordinate team activities
  • Interpersonal skills: support emotional, physical and social well-being of elders and Core Team
  • Problem solving / decision making
  • Technical expertise in Matrix Care, MS Office, and BluePrint
  • Walking, standing for lengthy periods of time.

Nice To Haves

  • Certified Dietitian/Nutritionist in NYS preferred
  • 1 year in long term care preferred

Responsibilities

  • Responsible for nutritional assessments and monitoring of assigned residents
  • Collaborate with clinical teams to facilitate appropriate nutritional care and care planning on an as needed basis
  • Provide diet counseling to residents and their family members as needed
  • Act as a liaison for policy and procedures pertaining to clinical nutrition
  • Serve as a liaison between foodservice and clinical nutrition needs; including menu updates, product updates and changes and coordination of therapeutic and mechanically altered dietary needs
  • Help with other duties as needed that support the Core Team to fulfill St John’s mission vision and values
