About The Position

San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. This is a Full Time position. Typical work schedule is Monday - Friday, 8:00 am to 4:30 pm. Job Summary: Under the supervision of the Dietitian Supervisor, the dietitian screens and assesses participant nutritional status for St. Paul’s PACE (SPP) participants. Uses pertinent data to plan and implement appropriate nutrition interventions and communicates the same to the interdisciplinary team to ensure the nutritional needs of the participants are met. Performs nutritional assessment and provides nutritional education for St. Paul’s PACE participants requiring interventions. Assists with and helps to coordinate serving of meals and monitors/coordinates kitchen facility to ensure standards are met. Assists with coordination with contracted meal services to ensure meals meet the needs of the St. Paul’s PACE participants. Provides education to other health care professionals as appropriate.

Requirements

  • Baccalaureate or advanced degree from an accredited college with major studies in food and nutrition or related field.
  • Must be a Registered Dietitian (RD) as confirmed by the Commission on Dietetic Registration.
  • Must be 18 years of age or older.
  • Minimum of 1 year’s experience working with a frail or elderly population both individually and in groups required.
  • Working knowledge of physical, mental and social needs of frail older adults.
  • Effective skills in assessment of nutritional needs of frail older adults.
  • Effective written and oral communication skills.
  • Ability to work with an interdisciplinary team, as well as independently.
  • Effective organizational skills.
  • Basic computer skills.
  • YOU MUST BE ABLE TO CLEAR FINGERPRINTS THROUGH DOJ AND FBI IN ADDITION TO A DRUG SCREEN CLEARANCE.
