About The Position

The Nutritionist II provides advanced nutrition care and counseling to intermediate-risk patients with cardiometabolic conditions, focusing on individualized dietary interventions to improve health outcomes and reduce risk factors. This role conducts comprehensive assessments, develops evidence-based nutrition plans, monitors progress, and collaborates with multidisciplinary teams to ensure integrated care.

Requirements

  • Bachelor’s degree from an accredited program through ACEND.
  • Registered Dietitian by the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics.
  • 0-2 years of experience as a registered dietitian.
  • Licensed in the state of employment as Dietitian/Nutritionist, if applicable; or willing to obtain license upon hire.
  • Registered Dietitian with the Commission on Dietetic Registration (CDR).
  • Basic knowledge of clinical nutrition principles and dietary management of common chronic conditions such as diabetes, hypertension, and obesity.
  • Knowledge of effective strategies for teaching and educating patients about general nutrition and dietary modifications.
  • Awareness of cultural and ethnic factors that may influence dietary choices and the ability to provide culturally sensitive nutrition advice.
  • Awareness of various health conditions and their dietary implications, especially in an outpatient setting.
  • Excellent communication skills, both verbal and written
  • Ability to create engaging educational materials and deliver presentations or classes to groups of patients.
  • Effective time management skills to balance teaching classes, conducting one on one sessions, and developing educational content.
  • Strong organizational skills to manage a varied caseload and maintain accurate documentation.
  • Strong customer service skills to provide patients with positive and supportive experience.
  • Willingness and enthusiasm for continuous learning and professional development in the field of clinical nutrition.
  • Bilingual English and Spanish is required.
  • Accepts working flexible schedules and locations if necessary.

Responsibilities

  • Conduct thorough nutritional assessments, considering medical history, dietary habits, and lifestyle, to create individualized nutrition plans for the intermediate risk population, mainly dealing with cardiometabolic conditions.
  • Perform physical evaluations to identify signs of malnutrition, including muscle wasting, edema, and possible skin changes like pressure injuries.
  • Conduct follow-up sessions with patients to assess progress, answer questions, and provide ongoing support in their dietary journey.
  • Collaborate effectively with other healthcare professionals, such as physicians, nurses, and coaches, to ensure coordinated patient care.
  • Maintain accurate and up-to-date patient records, including assessments, progress notes, and care plans, in accordance with regulatory standards.
  • Stay up to date with the latest research and trends in clinical nutrition to provide evidence-based information to patients and contribute to content development. Regularly review scientific literature, journals, and reputable sources for emerging nutrition research and guidelines, to enhance knowledge and expertise in clinical nutrition and related fields.
  • Assume a leadership position relative to newer dietitians, contingent upon the department's requirements at the time of recruitment.
  • Perform other duties as assigned by the Nutrition Manager, contributing to departmental projects and continuous improvement initiatives.
