About The Position
Incumbent is responsible for formulating, documenting and implementing nutrition care plans for Patients seen in assigned clinic. Duties include conducting and documenting assessments, reassessments, consults and providing nutrition education for Patients and Families. Participating in process improvement and special projects. Coordinating resources throughout the HealthSystem and assisting in the evaluation of services provided to ensure that requirements are met.
