About The Position

The Toledo Clinic is seeking a part-time Weight Management Registered Dietitian to join our Timberstone Family Practice. This position will be scheduled on Fridays from 9:00am - 2:00pm. The office is located at 4411 Holland-Sylvania Rd in Toledo OH. General Summary: The weight management Registered Dietitian supports the primary care physician/ referring provider by providing nutritional assessment, education and management to patients with chronic conditions who are at risk for disease progression and complications. In collaboration with the physician and interdisciplinary team, the Registered Dietician develops and implements an individualized care plan designed to promote the patient's understanding and management of their condition, optimizes quality of life, and improves clinical outcomes. The Registered Dietitian will also help to develop, implement, and assess nutritional programs for the weight management program.

Requirements

  • Consistently arrives at work, in professional attire, on time and completes all tasks within established time frame.
  • Seeks appropriate tasks when primary tasks are completed and assists co-workers as needed.
  • Demonstrates adaptability to expanded roles.
  • Must be a licensed Dietitian in the state of Ohio
  • Ability to coordinate with other healthcare professionals and staff
  • Must have exceptional problem-solving, critical-thinking, organizational, interpersonal, and written/verbal communication skills
  • Working knowledge of computer programs (email, Word, Excel, PowerPoint, etc.)
  • Willingness to comply fully with all organizational and departmental policies and procedures.

Nice To Haves

  • Experience in diabetes education and/or CDCES
  • Experience in weight loss clinic
  • RDN is strongly preferred

Responsibilities

  • Conduct comprehensive nutritional assessment, education, and management to patients referred by physicians and/or multidiscipline care team
  • Develop, implement, and coordinate individualized meal plans
  • Review and analyze patient's nutritional needs and goals to make appropriate dietary recommendations
  • Assess patients' knowledge and tailor education to fit patients' needs
  • Provide outpatient education and counseling to patients and families in individual or group situations, in accordance with established educational plans
  • Collaborate with multidisciplinary team and community professionals to ensure continuity of care
  • Communicate regularly with weight management program coordinator
  • Maintain timely clinical documentation in compliance with the Toledo Clinic, Inc. policies and regulatory standards
  • Obtain appropriate insurance information and properly code services provided
  • Maintain appropriate dietary status information on patients
  • Develop patient education resources
  • Refer patient to appropriate health care provider to assist patient with needs outside of the dietitian's scope of practice
  • Maintain at all times the confidentiality of the Toledo Clinic, Inc. proprietary information and patient medical, financial, or personal information, and generally any company information not made available to the public
  • Maintain at all times the confidentiality of former or current Toledo Clinic, Inc. patients' protected health information, as directed by state and federal law, and the Toledo Clinic, Inc. policy
  • Other duties as assigned.
