About The Position

As a Dialysis Social Worker at US Renal Care, you will play a crucial role in supporting patients with kidney disease. Requirements

• Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in the relevant state of practice.
• Experience in healthcare social work, particularly in dialysis or chronic illness management.

Responsibilities

• Conduct patient assessments and care planning activities, including psychosocial needs assessments and completing the KDQOL.
• Identify and counsel patients on psychosocial issues, providing education to patients and their families.
• Coordinate support communities for patients and families, identifying social agencies and resources for assistance.
• Collaborate with the interdisciplinary clinic team to promote teamwork and educate staff on patient psychosocial care.
• Participate in continuing education and staff meetings, maintaining positive relationships with local hospitals and agencies.
• Assist in clinical and operational processes to improve patient health and minimize missed treatments and hospitalizations.
• Participate in monthly Quality Assessment and Performance Improvement (QAPI) activities to ensure compliance with regulations.
