About The Position

Acute Dialysis Nurse is responsible for the delivery of patient care through the process of assessment, diagnosis, planning, implementation, and evaluation. This position participates in the development and implementation of individualized and comprehensive plan of care for each patient, provides direct and indirect care to patients as it relates to their dialysis treatment, including pre- and post-assessments, and administrations of medications during dialysis. In addition, the Acute Dialysis Nurse operates all dialysis equipment and supplies in accordance with hospital and departmental policy to provide expected patient outcome and documents in the medical record accordingly.

Responsibilities

  • Monitors and assures patients receive adequate medical attention prior to, during and after dialysis treatments.
  • Receives and accurately documents patient specific orders received from nephrologist.
  • Initiates patient therapy per physician order.
  • Recognizes changes in patients on dialysis and makes necessary adjustments in their therapy per physician order and appropriate interventions. Appropriately responds to all emergencies, notifies the nephrologist when necessary and seeks direction.
  • Sets correct priorities for patient care based on current knowledge. Consistently reviews and evaluates priorities as changes occur and conveys these priorities to appropriate staff members.
  • Administers medications when ordered. Understands the reason for giving medication and possible side-effects; ensures that medications are given properly, per physician order and documented.
  • Obtains all laboratory specimens as ordered
  • Verifies the accuracy of all orders during treatment.
  • Participates in facility continuing education and in-servicing.
  • Performs and documents all required machine testing, safety checks, set up, and disinfections.
  • Sets up and operates hemodialysis machines per policy and physician order.
  • Restock supplies and perform routine counts as directed by immediate supervisor.
  • Verifies correct dialysis machine settings according to physician order.
  • Monitoring of patient throughout the duration of the dialysis treatment.
  • Completes patient assessment pretreatment, during treatment as needed and post treatment.
  • Documents the administration of care in the patient’s medical record in a timely and thorough manner.
  • Assists in building and maintaining a facility/clinic culture of safety and quality.
  • Reports errors and near misses so that steps can be taken to prevent recurrence.
  • Reports any negative feedback, complaints, and grievances from other staff members, patients, and families.
  • Uses evidence-based guidelines and proper infection control practices.
  • Maintains open communication with Unit Manager, Medical Director, hospital management and staff.
  • Utilizes resources to improve skills, knowledge, and practice to assure continuous quality of patient care.
  • Teaches patients/families/caregivers about patient medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures
  • Teaches patients/families/caregivers about any non-medicinal follow-up measures, such as healthy diet and exercise, disease prevention, and/or other lifestyle changes as appropriate
  • Promotes a teamwork approach by offering information, advice, and assistance to all unit staff members in a positive, courteous, and cooperative manner.
  • Adheres to all company policies and procedures.
  • Performs other duties as required and/or assigned.
