  • CDCES Certified Diabetes Care and Education Specialist with three years’ experience.

Responsibilities

  • Provides education to patients.
  • Provides patient education to patient/family members/significant others about plan of care.
  • Follows appropriate communication channels with concern and recommendations.
  • Provides ongoing education to patient care on new procedures, technologies and issues impacting diabetes industry.
  • Performs assessments, planning and implementation of diabetes care measures and evaluation of patient outcomes across the continuum of care.
  • Collaborates with providers to facilitate patient care, develop medical plans of care and follows appropriate communication channels with patient concerns and recommendations.
  • Provides services to patients to ensure continuous quality improvement.
  • Participates in quality improvement process to facilitate changes within the clinic.
  • Remains aware of patient satisfaction results and problem solves with patients.
  • Identifies patient dissatisfaction and intervenes appropriately.
  • Preserves patient privacy and dignity.
  • Initiates development of new ideas and methods to provide cost-effective care.
  • Coordinates patient care services with other staff and departments to decrease unnecessary cost.
  • Assists providers in reducing costs and increasing effeciency while maintaining high quality of care.
  • Maintains diabetes supplies and medication appropriately.
  • Provide education and training to staff to ensure adequate knowledge and skills to perform essential responsibilities of their positions.
  • Perform other duties as assigned.
