As a Patient Educator, you will help coordinate the diabetes education program, providing comprehensive resources to physicians, nursing staff and patients. In this role, you will provide critical education and support on diabetes self-management, including glucose monitoring (with an emphasis on continuous glucose monitoring [CGM]), medication adherence, lifestyle modification and overall disease management. Working closely with the multidisciplinary care team, you will help develop, implement and monitor individualized care plans that promote optimal glycemic control and prevent complications. This position will facilitate organizational change by implementing services that support both individual and system-wide improvement, while delivering high-quality education that enhances organizational effectiveness, increases efficiency and strengthens our ability to meet and exceed patient and stakeholder expectations. It is also vital to maintaining the integrity of our diabetes program, ensuring regulatory compliance, supporting sustainable revenue growth and ultimately improving long-term patient health outcomes. A caregiver in this role works days from 8:00 a.m. - 5:00 p.m.
