About The Position

The Diabetes Nurse Specialist will work in the outpatient setting and inpatient setting as needed, acting as a member of the patient’s care team that participates in the continuum of care and education of patients with diabetes. Assess patient and pertinent family members’ knowledge of diabetes and develops and implements a patient-centered care plan; monitors and evaluates, as needed, resource utilization, patient outcomes and/or goals, patients’ skills in self-care, and ability/readiness to learn. The position communicates with providers regarding patient needs and ongoing status of care plan. The Diabetes Nurse Specialist provides clinical resource and consultation to staff and demonstrates advanced clinical competence in diabetes care.

Requirements

  • Active RN License, BSN or MSN preferred.
  • Certified Diabetes Educator (CDE) certification required within 90 days of hire.
  • Inpatient and outpatient nursing experience, with clinical expertise in diabetes care.
  • Clinical expertise in diabetes care with insulin pump/CGMS experience.
  • Working knowledge of American Diabetes Association (ADA) recognition for Diabetes Self-Management Education (DSME).
  • Experience in electronic medical records, data management and analysis preferred
  • Experience with individual and group teaching methodologies.
  • Registered Nurse Required at hire
  • Basic Life Support (BLS) through an American Heart Association (AHA) accredited course required at hire.
  • Certified Diabetes Educator Required within 90 days of hire
  • Integrative Agitation Management (IAM) Required within 30 days of hire
  • Demonstrated knowledge of education theory, principles and methods of adult and/or pediatric education.
  • Demonstrated knowledge of current American Diabetes Association (ADA). recommendation for patient management.
  • Demonstrated teaching and leadership abilities.
  • Demonstrated oral and written skills.
  • Demonstrated computer skills.
  • Demonstrated presentation skills.
  • Demonstrated ability to work collaboratively and develop positive interactions with patients, families, physicians, staff and other disciplines.
  • Ability to read, write, understand and follow English verbal and written instructions.
  • Ability to set priorities, achieve objectives, solve problems, meet deadlines, coordinate simultaneous projects, and make decisions.
