The position is responsible for organizing, educating, and scheduling patients for the Outpatient Diabetes Self-Management Program. The role includes providing consultations for patients and families regarding diabetes therapy, participating in instructing outpatient groups and one-on-one sessions, scheduling and registering outpatients for classes, documenting outpatient education in medical records, and developing teaching tools. The position also involves serving as a resource and clinical expert for identified diabetes inpatients, assisting with education for newly diagnosed patients, assessing needs of complicated diabetes patients, and consulting with medical staff as needed. Additionally, the role requires ongoing education of hospital staff regarding changes in diabetes management practices and maintaining professional growth through certifications and community activities.

  • Organize, educate, and schedule patients for the Outpatient Diabetes Self-Management Program.
  • Provide consultation for patients and families regarding diabetes therapy.
  • Participate in instructing outpatient groups and one-on-one sessions.
  • Schedule and register outpatients for classes as needed.
  • Document outpatient education in the patient's medical record.
  • Develop teaching tools and teaching aides as needed.
  • Serve as a resource/clinical expert for identified diabetes inpatients.
  • Assist with education of all newly diagnosed patients.
  • Assess needs of complicated diabetes patients per referral.
  • Reinforce education to patients/families as requested.
  • Make multi-disciplinary referrals as indicated.
  • Consult with Medical Staff as needed.
  • Provide training/education for monitoring equipment.
  • Document patient interventions in their medical record.
  • Provide input and guidance to nursing staff regarding education of diabetes patients.
  • Provide hospital staff with adequate supplies of Diabetes educational materials.
  • Ongoing education of hospital staff regarding changes in practice as it relates to diabetes management.
  • Obtain and maintain CDE certification.
  • Maintain professional education related to diabetes through readings, workshops, and professional memberships.
  • Maintain education file and participate in yearly evaluation by review date.
  • Plan and/or participate in any diabetes-related activities offered to the community.
  • Bachelor's degree in Nutrition.
  • 2-3 years nutrition experience with exposure to diabetes patients.
  • Experience with computer entry and retrieval.
  • Experience with insulin pump therapy.
  • Current RD license in the State of Illinois.
  • Specialty CDE certification within 18 months of hire.
