Requirements

• Certification as a Diabetes Educator by the American Diabetes Association.
• Registered dietitian in the state of Oklahoma OR Registered nurse in the state of Oklahoma.

Nice To Haves

• 3 years of experience as a dietitian or RN.

Responsibilities

• Develop diabetes self-management education and pre-diabetes related education resources for patients referred for consultations.
• Provide education and training for care managers, medical assistants and nursing staff regarding diabetes self-management using the 3 M of self-care and other care plans/resources in alignment with ADA guidelines.
• Assess clients' nutritional, health needs and social determinant needs.
• Conduct in person, Televideo and groups education and counseling.
• Participate in community events on the topic of healthy eating and diabetes education.
• Document care plans in Epic.
• Evaluate if the meal plans have a positive effect on clients' health.
• Create educational material about healthy eating habits.
• Speak to groups about the importance of nutrition to prevent specific diseases.
• Document patient progress.
