About The Position

Summary The role of the Diabetes Educator Outpatient setting is to lead / support improvement efforts that affect patients hospitalized with diabetes or hyperglycemia. This not only encompasses patient and family education, but education of interdisciplinary team members. The CDCES will also act as a lead/support to organizational achievement of diabetes-related quality metrics and performance outcomes. Diabetes self-management education and support is widely recognized as an important component of patient care, recovery, and reduced cost in the hospital setting. This position works collaboratively with interdisciplinary leadership and teams throughout the organization. Practice is in alignment with the organizational M/V/V and strategic initiatives. Requirements

  • BS required
  • Must hold board certification as Certified Diabetes Care and Education Specialist (CDCES).
  • Strong computer skills, competent in Word, Excel and PowerPoint.
  • Strong communication skills.
  • The ability to analyze and interpret regulatory guidelines and mandates.

Nice To Haves

  • MS preferred.
  • Experience in program development and previous leadership experience preferred.
  • Previous experience with project or program development preferred.

Responsibilities

  • Collaborates with Endocrine team to develop system improvement plans as needed that incorporate staff education for all disciplines as they interact in caring for patients and families with diabetes and/or hyperglycemia (providers, nursing, patient care technicians, dietary department members etc.)
  • Collaborate with health care team to develop safe processes to eliminate patient safety threats identified within the institution as it relates to glucose management and/or safe medication administration for patients with diabetes or hyperglycemia
  • Oversees and assists in the collection and analysis of glucometric data, sharing results with glucose management teams and leadership to guide development of quality improvement planning
  • Assists with protocol/order-set establishment guided by evidence-based practice, with primary goal to reduce rates of hypoglycemia and hyperglycemia in acute care setting and improving transition of care process
  • Establish partnerships with community-based providers, CDCES, and services to address diabetes self-care needs
  • Assist/perform chart reviews as needed to identify root cause for hypoglycemia/hyperglycemia or patient safety concerns identified
  • Provide input into patient self-management education planning at patient and institutional level (guide learning needs assessments, including health literacy and assisting with setting and prioritizing goals). The acute care setting focus is survival-skills teaching to bridge the transition to support team in the outpatient environment.
  • Support the development of processes in the acute care setting related to provision of education and documentation to ensure patients with diabetes are provided standardized survival- skills education
  • Consultant at bedside to assist/collaborate with multidisciplinary team (provider, bedside nurse, social worker, pharmacist, dietitian, and case management) in planning education for complex patients with diabetes or hyperglycemia
  • Works with others to identify problems and solutions
  • Uses an evidence-based approach to facilitate the initiation of, adoption of, and adaptation to change
  • Applies improvement processes to effect practice change
  • Exhibits creativity and flexibility through times of change
  • Participates in organizational committees and/or shared governance councils
  • Organize and lead unit-based Diabetes Resource Champion team
  • Co-chair diabetes steering committee and other glycemic related teams for system improvement related projects as identified in glucometric data analysis
  • Supports the integration of research into practice
  • Maintains professional practice knowledge base needed to help nurses prepare for contemporary nursing practice.
  • Maintains current knowledge and competency in CDCES practice.
  • Participates in educational activities related to area of specialty.
  • Uses feedback gained from self, peer, nurse, and administrative evaluation to improve role effectiveness.
  • Seeks experiences to develop, maintain, and improve competence in role.
  • Serves as a role model of professional nursing including, but not limited to, involvement in professional organizations, engagement in lifelong learning activities, dissemination of information through publications/presentations, and advocacy.
