You'll combine clinical expertise with compassionate teaching to help patients live healthier, more confident lives. Educate & Empower: Teach patients insulin self-administration, glucose monitoring, and use of glucose pumps. Support Lifestyle Change: Provide guidance on medications, nutrition, and lifestyle adjustments to improve outcomes. Lead Programs: Deliver DSME curriculum aligned with American Diabetes Association (ADA) standards and current best practices. Collaborate: Work closely with multidisciplinary providers to ensure coordinated, patient-centered care. Improve Outcomes: Collect and validate DSME program data to enhance effectiveness and patient success. Stay Current: Act as a resource for updated diabetes education and continuously refine program content.   • Graduate of an accredited school of nursing (BSN preferred).
  • Minimum of 5 years of clinical experience, including teaching in a specialty area.
  • Experience in outpatient care and working with multidisciplinary teams.
  • Strong critical thinking and clinical competence.
