About The Position

Provides diabetes self-management education/training (DSMT) and/or medical nutrition therapy (MNT) to individuals, family members, and groups. Manages the care of a distinct patient population through assessment of educational needs, nutritional guidelines, lifestyle interventional goals, and counseling. Coordinates patient care in collaboration with other healthcare providers (physicians, pharmacists, case managers, social workers, etc.) to develop effective individualized protocols (medication, therapy, nutrition, devices, etc.) that empower patients with techniques and behavior modifications to help control their disease, avoid potential complications, and bring about lasting improvements in their quality of life. ***Travel between sites required***

Requirements

  • Bachelor's degree in Dietetics, nutrition, or related field.
  • Registered Dietitian (RD) certification.
  • Basic Life Support (BLS) certification.
  • Proven basic competency in diabetes education specialty.
  • Demonstrated knowledge of diabetes monitoring and managing equipment.
  • Skilled in Motivational Interviewing and basic behavioral counseling.
  • Strong written and verbal communication skills.
  • Ability to work collaboratively in diverse teams.
  • Knowledge of adult learning theory.

Nice To Haves

  • Experience in a diabetes clinic or endocrinology setting.
  • Advanced knowledge of adult learning theory.
  • Experience with electronic medical records (EMR) systems.

Responsibilities

  • Provide diabetes self-management education/training (DSMT) and/or medical nutrition therapy (MNT).

  • Gather required hours to become eligible for and sit for the CDCES exam within 2 years of hire.

  • Collaborate and extend care beyond provider visits.
