The Diabetes Educator Dietitian (RD CDCES) is accountable for his/her own professional development and practice within the scope of care defined by law. Utilizing organizational resources, the RD CDCES acts as a patient advocate for the delivery of safe care. Provides diabetes self-management training (DSMT) and medical nutrition therapy (MNT) to patients with Type 1, Type 2, or Gestational Diabetes in the specialty physician practice setting. Participates in the interdisciplinary care of patients including assessing and monitoring the diabetes management status, nutritional status, developing and implementing individualized care plans, and educating on diabetes self-management, nutritional interventions, therapeutic diets and nutritional needs. Works collaboratively with the interdisciplinary medical practice staff to promote glycemic control for patients. The Diabetes Educator Dietitian reports to the Administrative Director of Endocrinology.
