As a Diabetes Educator Dietitian, you will provide specialized nutrition education and counseling for individuals with diabetes, empowering self-management. Every day you will assess patient needs, develop personalized meal plans, educate on glucose management and lifestyle, and conduct sessions applying evidence-based practices. To be successful, you will demonstrate expert knowledge in diabetes nutrition, excellent teaching skills, and a compassionate approach to optimize patient health outcomes. Coordinates the planning, implementation and evaluation of the DSMES program. Measures the effectiveness of the DSMES through the development and utilizations of a Continuous Quality Improvement (CQI) plan. Responsible for maintaining ADA Recognition and participating in the evaluation of the program. Develops and maintains an education curriculum, including selection and on-going review of educational materials to ensure they meet the needs of the population targeted. Develops and maintains policies/procedures/standing orders as they relate to inpatient and outpatient education. Provides education following the ADA curriculum framework of these 9 topic areas: diabetes pathophysiology; healthy eating; being active; taking medication; monitoring glucose; acute complications - prevention, detection and treatment; chronic complications - prevention, detection and treatment; lifestyle and healthy coping; and diabetes distress and support for both inpatients and outpatients.
