About The Position

We are searching for a Diabetes Educator III. Someone who provides assessment, goal setting, education and behavioral intervention, and on-going case management to patients and caregivers to promote diabetes self-management in the inpatient and ambulatory settings. Contributes to the development of position papers, guideline development, or other literature to advance the specialty. Develops processes to improve transitions of care and ensure appropriate self-management education and resources. Creates innovative strategies to achieve the Quadruple Aim (population health, quality of patient care experience, costs associated with care, and meaningfulness of the provider's experience). Texas Children’s Pavilion for Women is a comprehensive obstetrics/gynecology facility and one of a select few hospitals nationwide that provides women, mothers and babies with a full continuum of high-quality, expert health care. Offering a full spectrum of maternal and fetal medicine services, including an array of fetal diagnostic procedures and highly specialized fetal surgeries, the Pavilion for Women is connected to Texas Children’s Hospital via a two-story circular sky bridge to enhance patient care by providing physicians, staff and patient families with rapid access to other pediatric subspecialists. Three private OB/GYN practices, The Family Fertility Center, The Menopause Center and The Women’s Place – Center for Reproductive Psychiatry are all housed here. Additionally, we also recently opened a community-based OB/GYN practice in Pearland.

Requirements

  • Associate's Degree Nursing, Bachelor's Degree, or Master's Degree in Nutrition, Public Health, Education, or Nursing
  • CDCES - Cert Diab Care and Educ Spec Certification Board of Diabetes Care and Education Or BC-ADM - Cert-BC Adv Diabetes Mgmt Certification Board of Diabetes Care and Education
  • 5 years Diabetes care as a licensed Dietitian or Nurse required

Responsibilities

  • Provides individualized diabetes self-management education and training to individuals and groups according to the Scope of Practice, Standards of Practice, and Standards of Professional performance for Diabetes Educators.
  • Uses the ADCES 7 Self-Care Behaviors® as a framework to teach, reinforce, and evaluate self-management skills.
  • Assesses person’s skills and knowledge level, education, health literacy/numeracy, culture, readiness to learn, preferred learning style and language, barriers to learning, financial barriers, confidence and skills with technology use, and support needs.
  • Records daily activities and patient related encounters using appropriate formats within the electronic medical record.
