Promotes efficient and effective service delivery through enhanced communication and coordination of the patient specific plan of care. Ensures the established standard of care and expected outcomes are achieved in a timely manner while facilitating appropriate resource utilization and enhancing customer satisfaction. Monday-Friday flexible schedule to meet patient care needs.

Requirements

  • Bachelor's Degree from an accredited college or university in nursing Required
  • Minimum 3 years of nursing experience required Required
  • Current and valid Registered Nurse Nebraska/Compact State licensure Upon Hire Required
  • Current and valid Basic Life Support (BLS) through the American Heart Association Upon Hire Required
  • Certified Diabetic Educator (CDE) certification within 3 years of hire. Required
  • Must be able to perform a variety of duties, often changing from one task to another of a different nature, without loss of efficiency or composure, and be able to perform under stressful conditions when confronted with multiple requests, demonstrating prioritizing

Responsibilities

  • Conducts holistic age-specific patient/family educational needs assessments according to defined standards set by the American Diabetes Association and the unit’s standard of care. Completes on-going educational needs assessments/reassessments according to the appropriate CPG or protocols, unit standard of care, and American Diabetes Association guidelines. • Collects and documents data related to patient’s physical, mental, developmental, emotional and social condition. • Demonstrates age-specific competency in assessment of patients by having the ability to obtain and interpret information in terms of the patients’ needs, knowledge of growth and development, and understanding the range of education needed by these patients. • Assesses family expectations for, and involvement in the assessment and continuous care or treatment of the patient at home. Meets families’ expectations related to home diabetes management through educational preparation during their hospitalization. • Communicates to the patient/family the plan of care related to in-patient diabetes education. Coordinates mutually agreeable times with appropriate care givers to provide education while hospitalized. • Identifies the need for MD involvement or a potential change in plan of care based on patient condition/family needs. • Reviews/revises CPGs, teaching sheets, and computerized order sets to meet ADA/evidenced-based practice standards. Develops unit standards which align with these practices standards. Promotes the implementation of practice standards throughout in-patient settings.
  • Education • Considers cultural, cognitive, emotional, and social influences on the patient/families ability and readiness to learn. • Develops appropriate educational goals with patient/family. Documents goals in the in-patient chart and out-patient record. • Provides education to patient/family at an appropriate level for their understanding. Recognizes the magnitude of information needing to be received by patient/family and continually assesses their understanding of information presented. Tailors education to the individuals being taught, yet utilizes a systematic approach to present information ensuring all applicable content is addressed. • Ensures all teaching is documented on the in-patient and the out-patient FERs to include understanding of topics taught. • Develops, implements, and evaluates staff orientation & ongoing education related to care of the pediatric diabetic patient as needed.
  • Care Coordination • Provides timely communication to/from patient/family and other healthcare disciplines; answers patient care related questions; facilitates staff education at bedside; makes requests for new orders or order adjustments, etc. • Promotes coordination of care by rounding with attending physician, resident staff (if appropriate), and with consulting Endocrinologist. Serves as a liaison between medical providers by discussing current treatment plan and proposed recommendations for changes to the plan of care. • Promotes coordination of care by assuring completion of tests, reviewing current orders for applicability, and developing a daycare/school plan. Collaborates with social services for high risk patients/families needing emotional, financial and/or social support. • Prepares patient/family for discharge by providing adequate supplies/materials, facilitating the submission of prescriptions, and the scheduling of out-patient education (i.e. class) and follow-up appointment with Endocrinology provider. • Facilitates continuity of care by attending Endocrinology weekly team meetings to discuss hospitalized patients. Offers key information related to patient/family dynamics, successes/challenges encountered during education, and the extent of anticipated out-patient support needed by patient/family.
