Requirements

  • Experience in diabetes education and management.
  • Knowledge of pathophysiology, epidemiology, and clinical management of diabetes.
  • Ability to apply evidence-based care and current research.
  • Strong communication and collaboration skills.
  • Experience in quality improvement processes.

Nice To Haves

  • Registered Dietitian certification.
  • Experience with technology-enabled diabetes education.

Responsibilities

  • Reliable and punctual attendance is essential.
  • Communicate necessary information to others as appropriate.
  • Integrate skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions, and self-management of diabetes into clinical practice.
  • Advocate for and communicate about improved quality of care and outcomes for those living with, at risk for, and affected by diabetes and cardiometabolic conditions.
  • Actively participate in the quality improvement (QI) process and adapt practice/process based on QI findings.
  • Partner with individuals and caregivers to deliver care and education conducive to behavior change and improved quality of life for self-management of diabetes and cardiometabolic conditions.
  • Apply current research and evidence-based care to practice.
  • Apply business principles, systems practice, and population health management to support achievement of the Quadruple Aim.
  • Apply the AADE7 self-care behaviors to educate on and initiate behavior change.
  • Engage in lifelong learning and serve as a role model of professionalism.
  • Provide quality diabetes self-management education and medical nutrition therapy in individual and group settings.
  • Utilize appropriate teaching techniques sensitive to the learning preferences of the person with prediabetes or diabetes.
  • Follow the ADCES Diabetes Education Accreditation Program requirements.
  • Complete comprehensive assessments for each patient and develop individualized care plans.
  • Document all individual contacts/visits in the Electronic Health Record in a timely manner.
  • Collaborate with other members of the diabetes care team in developing person-centered diabetes plans.
  • Advocate for and support technology-enabled diabetes education and care.
  • Collaborate with community partners to establish and maintain ongoing support options.
  • Contribute to the achievement of established department goals and objectives.
  • Comply with governmental and accreditation regulations.
  • Participate in meetings, serve on committees, and represent the department in community outreach efforts.
