About The Position

The diabetes care and education specialist (DCES) delivers comprehensive and seamless services that bridge the gap and integrate clinical and self-management aspects of diabetes care. The DCES is an integral part of the interprofessional team and provides collaborative, comprehensive, and person-centered care and education conducive to behavior change and improved quality of life across the lifespan. The DCES delivers case management services to patients to ensure cost-effective use of resources and positive patient outcomes. The DCES supports and advocates for people affected by diabetes to optimize quality care. Integrates skills and knowledge of pathophysiology, clinical management, motivational interviewing, and self-management of diabetes and the American Diabetes Association’s Standards of Medical Care in Diabetes to clinical practice. Applies the ADCES7 self-care behaviors to educate patients and initiate behavior change. Incorporates the ADA and/or ADCES Diabetes Education Accreditation Program requirements which are based on the National Standard of Diabetes Self-Management and Support Guidelines. Provides quality diabetes self-management education and medical nutrition therapy (limited to Registered Dietitians) in individual and group settings, in person or via telehealth, based on assessed needs. Utilizes teaching techniques that are age-appropriate and sensitive to the learning preferences of the person with prediabetes, gestational diabetes, or diabetes. Advises Health Care Providers as needed regarding diabetes medications, diabetes treatment plans, and diabetes services. Provides case management, including titration of insulin and titration of GLP-1 agonist receptor medications. ​​​​​​​​​​​​​​The tasks outlined above are intended to provide a general overview of the role and are not an exhaustive list of all duties, responsibilities, or job functions. Requirements

  • Licensed by the State of Nebraska as a Registered Nurse required, OR Registered Dietitian certification through the Commission of Dietetic Registration required.
  • If not already DCES will sit for the certification examination within 12 months of hire date.
  • Minimum of one year of experience working as a Registered Nurse or Registered Dietitian required.
  • Proficiency in both English and Spanish required.
  • Proficient computer skills in Microsoft Office products (Word, Excel, PowerPoint, etc.) required; prior experience with an Electronic Health Record preferred.
  • BLS certification required for Registered Nurses and Registered Dieticians in this role.
  • Demonstrated ability to manage multiple tasks.
  • Demonstrated ability to work with minimal supervision independently and as a team member.

Nice To Haves

  • Diabetes self-management education and support experience preferred.
  • Expertise in diabetes, nutrition, physical activity, motivational interviewing, weight reduction, cholesterol management, stress management, and/or hypertension management preferred.
