About The Position

The position involves assessing learning needs, developing education plans, providing education, and evaluating the outcomes of education for patients and caregivers with diabetes. The role includes providing education in both inpatient and outpatient settings, participating in policy and protocol development teams, and serving as a resource for staff education. The position may also oversee the day-to-day operations of Diabetes Self-Management Education and Support (DSMES) programs, ensuring compliance with National Standards for DSMES.

Requirements

  • Graduate of an accredited school of nursing, pharmacy, or registered dietitian.
  • Basic Life Support Healthcare Provider (BLS HCP) certification required within 90 days of employment.
  • State-specific licensure as a Registered Nurse, Pharmacist, or Registered Dietitian.
  • Certified Diabetes Educator (CDE) certification from the National Certification Board for Diabetes Educators (NCBDE).

Responsibilities

  • Assess patient/family/caregiver baseline knowledge about diabetes and readiness to learn.
  • Provide diabetes and self-management education to patients, families, and caregivers.
  • Monitor and evaluate patient responses to therapeutic interventions in collaboration with the healthcare team.
  • Coordinate with staff to integrate medical therapy into diabetes management.
  • Utilize health literacy principles to develop and deliver effective diabetes education.
  • Participate in continuous improvement by monitoring trends in hypoglycemia/hyperglycemia.
  • Oversee planning, implementation, and evaluation of DSMES programs.
  • Coordinate activities of the Advisory Group and act as a liaison between DSMES staff and other departments.
  • Monitor and facilitate maintenance of DSMES staff qualifications.
  • Maintain ADA Recognition and evaluate program effectiveness.
  • Perform other duties as assigned.
