The Diabetes and Cardiovascular Project Coordinator supports Maine CDC’s Chronic Disease Prevention and Control Program by facilitating, implementing, and coordinating initiatives within the Cardiovascular Health (CVH) and Diabetes Prevention Programs. This role also supports Maine CDC’s Rural Health Transformation Program (RHTP) by deploying evidence‑based practices that advance population health across clinical and community settings. The successful candidate will collaborate closely with internal program staff, healthcare providers, and community partners to ensure timely implementation of federally funded work plans, high‑quality grant reporting, and continuous improvement of chronic disease prevention strategies statewide.
