About The Position

The Diabetes and Cardiovascular Project Coordinator supports Maine CDC’s Chronic Disease Prevention and Control Program by facilitating, implementing, and coordinating initiatives within the Cardiovascular Health (CVH) and Diabetes Prevention Programs. This role also supports Maine CDC’s Rural Health Transformation Program (RHTP) by deploying evidence‑based practices that advance population health across clinical and community settings. The successful candidate will collaborate closely with internal program staff, healthcare providers, and community partners to ensure timely implementation of federally funded work plans, high‑quality grant reporting, and continuous improvement of chronic disease prevention strategies statewide.

Requirements

  • Bachelor’s degree in public health, health, or a related field or a combination of four (4) years of relevant education, training, and/or experience.
  • Demonstrated ability to work collaboratively with diverse partners and stakeholders.
  • Strong organizational, time‑management, and prioritization skills.
  • Ability to exercise sound judgment in complex, multi-stakeholder environments.
  • Excellent written, verbal, and interpersonal communication skills.
  • Ability to manage multiple projects simultaneously and meet deadlines.
  • Proficiency with Microsoft Office (Outlook, Word, Excel, PowerPoint, Publisher).
  • Demonstrated cultural competence and professionalism.

Nice To Haves

  • Master’s degree in a related field (public health, health administration, population health, or similar).
  • Experience working with federally funded grants or public health programs.
  • Experience supporting quality improvement or practice facilitation initiatives.
  • Experience working with healthcare systems or community-based organizations.

Responsibilities

  • Coordinate and implement federally funded work plans in collaboration with the Program Manager and Coordinators.
  • Provide content for annual work plans and progress reports within federal grant reporting systems.
  • Offer subject matter support related to cardiovascular disease and diabetes prevention initiatives.
  • Deliver technical assistance to healthcare providers and evidence‑based program partners to support prevention and self-management programs.
  • Participate in learning collaboratives and stakeholder workgroups to advance action plan activities.
  • Contribute to strategic discussions related to funding opportunities, legislative monitoring, and program planning.
  • Attend regularly scheduled meetings with program staff, partners, contractors, and stakeholders.
  • Apply strong customer service practices in all interactions with partners, colleagues, and the public.
  • Implement grant work plans, budgets, and contracted activities focused on evidence‑based chronic disease prevention strategies.
  • Support completion of required grant reporting and deliverables within established timelines.
  • Facilitate coordination with contracted partners and stakeholders to support work plan objectives.
  • Participate in department‑wide planning efforts that advance a collaborative, population-health approach across Maine.
  • Coordinate project deliverables, including reporting, communications, quality improvement activities, and data-informed program enhancements.
  • Maintain and strengthen partnerships with key contributors and organizations.
  • Support integration of evaluation findings into ongoing program operations.
