Location: Westside Dental Center, 120 King Street, Jacksonville, FL 32204. Hours: Monday-Friday, 8:00am-5:00pm, 2-3 days per week. Computer Software : EagleSoft Electronic Oral Health Record and HMS (Health Management System). Dress Code : Medical Scrubs. Must Be Fully Enrolled as a Florida Medicaid Provider

Requirements

• Must be a fully enrolled Florida Medicaid Provider
• Credentialed with DentaQuest Dental Insurance
• Credentialed with Liberty Dental Insurance
• 1 year experience provide a full range of general dental services
• DDS or DMD degree
• Active Florida Dental License

Responsibilities

• Providing a full range of general dental services including comprehensive dental exams
• preventive services
• restorations
• removable complete and partial dentures
• extractions within the scope of a general dentist
