About The Position

Thriving non-profit, federally qualified health center headquartered in Palatka, Florida is seeking a motivated and experienced dentist. Competitive salary and comprehensive benefits. EOE. This is a full-time position. Monday-Thursday,8:00 am-7:00 pm   POSITION SUMMARY Provide the full scope of primary care oral health services which fall under his/her field of training, including, but not limited to, diagnosis, treatment, coordination of care, preventive care and health maintenance to patients of Aza Health (AH). In addition, the Dental Provider is a member of the AH team. In that capacity, he/she is expected to be familiar with the philosophy, goals and objectives of AH and work collaboratively to achieve them. He/she must be cognizant of, and comply with, all AH policies and procedures, as well as pertinent state and federal regulations. This position reports to the Chief Dental Officer.

Requirements

  • Dental Degree (D.D.S. or D.M.D.)
  • State of Florida Licensure
  • Basic Life Support Certification
  • Computer literate - ability to utilize electronic health record in the performance of clinical duties.
  • Ability to analyze complex information and communicate effectively both orally and in writing.
  • Excellent interpersonal skills to establish effective and harmonious relations with AH patients, AH staff and the general public.
  • Commitment to and ability to successfully demonstrate honesty, confidentiality, integrity, quality, teamwork, customer service, and professionalism. Must have and display a positive outlook. Must demonstrate dedication to the Aza Health mission and to the success of the corporation.

Nice To Haves

  • Three to five years preferred, but not required
  • Language – Bilingual (English/Spanish) preferred, but not required.

Responsibilities

  • Provide direct dental services to AH patients to include diagnosis and treatment of injuries, diseases and malformations of teeth, gums and related oral structures (obtain health history, perform exam, document treatment plan, minor surgical procedures, etc.); preventive and maintenance care (regular oral exams, preventive services, health education, etc.); and, specialty care referrals. May treat diseases of nerve, pulp and other dental tissues affecting the vitality of the teeth. Patient care must be completed in a manner consistent with applicable clinical care guidelines.
  • Work no less than 40 hours a week in the provision of outpatient primary dental care to AH patients.
