About The Position
Dentist - DDS/DMD - California. Facility City & State: Crescent City, CA. Facility Type: FQHC. Provider Profession Needed: DDS, DMD. Specialty: General Dentistry. Start Date: May 23, 2026. End Date: 3 month contracts minimum. Length of Assignment: 3-month assignment with potential to extend. Prefer 6+ month contracts. Credentialing Timeframe: 3-4 weeks. Work Schedule: Monday through Friday OR 4 10/hr days. Patient Volume: Average of 18-20 patients per day. MUST see 8-9 minimum patients per 4 hours. Procedures: General Dentistry - Bread n butter - Must see all ages: 50/50 kids/adults. Extractions are a MUST. EMR: EPIC. Licensure Needed: CA. Certifications Needed: DEA and BLS - Board Certified. Relevant Experience/Skill Set Needed: MUST have 3 years of experience. Comfortable working in rural health. Interview Schedule/Preferences: Name clear. Presentation then reviewed my Med Director/Site Director. Zoom call and offer. Other additional pertinent information: For credentialing: MUST have 3 references within 2 years. Client runs own background check/NPDB. Must be up to date on ALL immunizations (COVID isn't required anymore) and if not up to date on immunizations, MUST wear mask all day in clinic. Do NOT have to be Denti-Cal enrolled, WILL get enrolled during credentialing
Requirements
- DDS or DMD
- CA License
- DEA and BLS - Board Certified
- 3 years of experience
- Comfortable working in rural health
- Must see 8-9 minimum patients per 4 hours
- Must see all ages: 50/50 kids/adults
- Extractions are a MUST
- Must have 3 references within 2 years
- Must be up to date on ALL immunizations (COVID isn't required anymore) and if not up to date on immunizations, MUST wear mask all day in clinic
