About The Position

Tepeyac understands that treatment of the whole person includes medical, behavioral health and dental care. All three are needed to achieve the utmost quality care and coordination for patients. Dental decay is the number one chronic disease in children that is 100% preventable —5 times more common than asthma, 4 times more common than early childhood obesity, and 20 times more common than diabetes. Decay can affect speech and communication, eating and dietary nutrition, sleeping, learning, playing and quality of life, missed school and/or work for patients of all ages. The Dental Hygienist works with a team of dentists and dental assistants to provide quality dental care to Tepeyac’s patients.

Requirements

  • Accredited dental hygiene school graduate.
  • Oral and written fluency in English and oral fluency in Spanish required.
  • Maintain a valid Dental Hygienist License with the State of Colorado.

Responsibilities

  • Work with dentist and dental team to identify best practice model for integration of preventive dental services with medical and behavioral health services.
  • Work with dentist to create referral process and relationships for basic general practice level dentistry as needed to diagnose and treat oral health needs identified during the provision of preventive services.
  • Work in collaboration with dentist to establish short-term and longer-term benchmarks for number of services provided and number of patients seen, by payer type.
  • Document the above documents in a Tepeyac Preventive Dental Services Plan.
  • Provide treatment/services to patients.
  • Provide preventive dental services to Tepeyac patients.
  • Maintain electronic patient records consistent with industry standards, and as needed to facilitate billing.
  • Complete chart notes within 3 days of service provision.
  • Provide preventive dental services in community settings periodically as agreed upon with Tepeyac Community Health Center. Will participate in school-based dental services and/or HUB sites.
  • Facilitate, with assistance from referral coordinator and case manager, proper referral coordination for patients.
  • Submission and review of weekly progress report with staff dentist.
  • Achieve monthly patient and service provision benchmarks as identified in the Preventive Dental Services Plan.
  • Attend required internal meetings, trainings, and events.
  • Other duties as assigned.
