About The Position

As a Respite Provider within the Guiding an Improved Dementia Experience (GUIDE) Model, you will provide compassionate, person-centered care to individuals living with dementia while offering meaningful respite and support to family caregivers. This role focuses on safety, dignity, engagement, and emotional support in the client’s home or designated respite setting. About the GUIDE model: Purpose: The GUIDE Model is a CMS-led initiative aimed at improving dementia care through comprehensive, coordinated care, caregiver support, and enabling people with dementia to remain in their homes and communities. Timeline: The model began on July 1, 2025, and will run for seven years. Focus: The model aims to improve quality of life for people with dementia, reduce caregiver strain, and enable people with dementia to remain in their homes and communities.

Requirements

  • Current Certified Nursing Assistant (CNA) certification. Must be active on the Iowa CNA registry.
  • Experience working with individuals with dementia and their families.
  • Must demonstrate empathy, compassion and a genuine interest in supporting individuals with dementia and their caregivers.
  • Ability to remain calm, patient, and compassionate in challenging situations.
  • Strong communication and interpersonal skills.
  • Ability to work independently in a home setting.
  • Current Iowa driver’s license, reliable vehicle, and car insurance required.
  • CPR certification (or willingness to obtain).
  • Ability to move 50lbs with our without accommodation
  • In a 10 hour day, employee may sit 1 to 5 hours, stand or walk 3-7 hours, including drive time.
  • Pre-employment/post-offer physical, TB, and drug screen required.

Responsibilities

  • Provide hands-on personal care, including bathing, dressing, grooming, toileting, and mobility assistance as needed.
  • Offers supervision and companionship to ensure client safety and comfort.
  • Engage clients in appropriate cognitive, social, and recreational activities tailored to their abilities and interests.
  • Support clients with dementia-related behaviors using patience, redirection, and evidence-based care techniques.
  • Assist with meals, hydration, and light housekeeping related to client care.
  • Monitor and report changes in client condition, behavior, or safety concerns to the GUIDE team per policy.
  • Provide emotional support and reassurance to both clients and family caregivers.
  • Follow all care plans, agency policies, confidentiality standards, and infection control procedures.
  • Accurately documents care provided and observations per agency guidelines.
