About The Position

We’re hiring a Nurse Practitioner (NP) or Physician Assistant (PA) to support our dementia care program . This hybrid role combines clinic-based and telehealth visits, with opportunities to expand into senior living communities. You’ll conduct cognitive evaluations, manage behavioral and chronic conditions, and educate families navigating dementia. This is a new and pioneering role—perfect for a provider who is comfortable with ambiguity, thrives in evolving environments, and wants to be part of building something meaningful from the ground up.

Requirements

• Active NP or PA license (in good standing)
• Passion for memory care, geriatrics, and patient connection
• Strong communication and collaboration skills

Responsibilities

• Conduct 45-minute cognitive assessments (CPT 99483)
• Deliver dementia-focused care via clinic and telehealth
• Coordinate with PCPs and care partners
• Manage follow-up visits and documentation
• Support senior living expansion initiatives
