About The Position

The Dementia Care Specialist is responsible for providing support, education, and outreach to various community stakeholders to promote dementia-friendly communities. This role focuses on assisting the elderly and individuals with disabilities, mental illness, or substance abuse disorders to live with dignity and security, while maximizing their self-sufficiency and choices. Additionally, the Specialist will coordinate the National Family Caregiver Support Program as outlined in the Older Americans Act.

Requirements

  • Bachelor's degree in Arts or Science, preferably in a human service-related field, or a license to practice as a registered nurse in Wisconsin.
  • Prior work experience in social services or human services working with older individuals and/or those with disabilities is preferred.
  • Valid driver's license is required.

Responsibilities

  • Provide support and education to businesses, churches, organizations, families, and individuals regarding dementia awareness.
  • Promote the development of dementia-friendly communities.
  • Assist elderly individuals and those with disabilities, mental illness, or substance abuse disorders in experiencing daily life with dignity and security.
  • Maximize opportunities for self-sufficiency and choice for clients.
  • Coordinate the National Family Caregiver Support Program as designated through the Older Americans Act.
