About The Position

Implements the Virginia Dementia Care Management (DCM) program in collaboration with the Dementia Care Management (DCM) Navigator. Serves as qualified resource for Dementia education, care coordination and advocacy within the Agency. Supervision: Performs work under the direct supervision of the Dementia Care Management (DCM) Program Manager.

Requirements

  • Strong knowledge of dementia care practices, aging, and neurological disorders.
  • Knowledge of aging and/or the impact of disabilities and illness on aging; local human and health service delivery systems, including support services.
  • Ability to demonstrate a positive regard for individuals, their caregivers, and their families; be persistent and remain objective; work as a team member, maintaining effective inter-and intra-agency working relationships; work independently, performing position duties under general supervision; communicate effectively, verbally and in writing; and develop a rapport and to communicate with persons from diverse cultural backgrounds.
  • Ability to facilitate group learning, public speaking, and community engagement.
  • Graduation from an accredited college or university with major course work in human services, gerontology, or a related field is required.
  • The employee must pass background check (Central Criminal Records Exchange (CCRE) through the Virginia State Police).

Nice To Haves

  • Two years’ experience in direct services human services with dementia care focus is preferred.

Responsibilities

  • Supports referral and service delivery of The Span Center programs for DCM clients and VCU GUIDE Clients
  • Performs initial intake screenings for DCM program from community, accepts internal referrals from agency to assess for DCM program eligibility.
  • Works closely with DCM Navigator and VCU GUIDE team to ensure comprehensive and robust support for dementia dyads (person living with dementia and their care partner)
  • Delivers Dementia Friendly training, DCM outreach, and dementia education to promote awareness, risk reduction, encourage early detection, brain health, and dementia resources throughout the community
  • Works with the Caregiver Support program to develop and implement dementia-specific caregiver skills training (communication, home safety, community resources, advance care planning, senior law days, etc.)
  • Provides Caregiver Support resources to caregivers.
  • Maintains client reporting for DCM clients in PeerPlace and other DARS required reports, including creating profiles of referrals, documenting contact units, and uploading care plans.
  • Works with DCM Navigator and Manager to ensure compliance with DCM Standards in delivery and administration
  • Actively participates in staff professional development programs.
  • Attends meetings and training sessions to enhance knowledge and skills.
  • Performs other duties as assigned.
  • Participates in advocacy efforts to support and address the diverse needs of the communities we serve.
