The purpose of Delivering Change Navigator is to reduce infant mortality; and improve the health and wellbeing of Geary County families through centralized and coordinated community collaboration. Delivering Change: Healthy Families, Healthy Communities is committed to improving the health and well-being of local prenatal, postpartum and childbearing aged women, infants and children, men and families through direct navigation services.

  • Provide case management services to pregnant, postpartum and inter-conception women, infants and families.
  • Provide education and support to women before, during and after their pregnancy.
  • Provide social determinates of health screenings, referrals for services and appropriate follow up.
  • Provide perinatal mental health screenings, education, and referrals for services and appropriate follow up.
  • Build relationships with patients, healthcare providers, agencies and other resources to best connect women and their families to helpful services.
  • Assist families with enrollment into other programs and resources, such as KanCare, WIC, KanQuit, Universal Home Visiting, etc.
  • Facilitate referrals for clients who need further assistance with medical and social resources.
  • Follow up with patients/clients regarding appointments/referrals, identifying barriers to care and assisting in solving those barriers.
  • Participate in program activities available to families.
  • Document all appointments, referrals, case management encounters, etc., in the appropriate data system and/or electronic medical records, relevant to individual patients.
  • Assist with enrollment, education, outreach and data collection of Becoming A Mom Prenatal Education classes.
  • Assist with teaching Becoming a Mom classes throughout the year.
  • Network with other agencies, programs and partners to ensure they understand services and support available to local families.
  • Understand local programming services available to families and healthcare offices’ referrals process.
  • Attend meetings, webinars and trainings.
  • Participate in Delivering Change Board Meeting, Geary County Coalition Meetings and other meetings as assigned.
  • Keep patients medical and social information confidential.
  • Comply with Stormont Vail Health policies and standards.
  • Other duties as assigned.
  • Complies with all policies, standards, mandatory training and requirements of Stormont Vail Health
  • Performs other duties as assigned
  • Bachelor's Degree Social Work, Family Studies or a related field. 3 years of working in case management may be substituted for education.
  • Lactation education.
  • Understanding of community health care systems and resources, data analysis methods; organizational and interpersonal skills and excellent skills in working with the cultural diversity of the community. (Required proficiency)
  • Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team. (Required proficiency)
  • Maintain confidentiality in all areas of the service population and program operations. (Required proficiency)
  • Work collaboratively with other staff members, service providers, and professionals. Excellent interpersonal skills with patients, colleagues, community leaders and policymakers. (Required proficiency)
  • Knowledge of prenatal care, breastfeeding, infant and child development. General understanding of medical terminology. (Required proficiency)
  • Excellent time management skills; organized and able to prioritize. Comfortable in a fast-paced environment with multiple cases. (Required proficiency)
  • 3 years Experience in case management. A Bachelor's degree in Social Work, Family Studies or a related area may be substituted for experience.
