About The Position

The Delegating Nurse at Broadmead, Inc. plays a crucial role in providing clinical oversight and ensuring the well-being of residents in an assisted living and memory care environment. This part-time, PRN position involves conducting assessments, managing medication protocols, and collaborating with various stakeholders to deliver high-quality care. The nurse will also participate in the holistic wellness approach of the facility, contributing to the overall health and lifestyle of the residents.

Requirements

  • Registered Nurse (RN) license required.
  • Experience in assisted living or memory care settings preferred.
  • Ability to perform assessments and manage medication protocols effectively.
  • Strong communication and interpersonal skills to interact with residents, families, and healthcare professionals.

  • Knowledge of state regulations regarding assisted living and memory care.
  • Familiarity with electronic health records (EHR) systems.

Responsibilities

  • Performs initial and 45-day assessments on-site and at a minimum of every forty-five (45) days.
  • Assesses the competency of certified medicine aides and certified medicine techs every 45 days.
  • Assesses the tasks delegated to determine if it is still reasonable and prudent to delegate every 45 days.
  • Assesses residents and medications on admit or readmission within 14 days according to state regulations.
  • Assesses residents and medications on all self-medicating residents every 90 days.
  • Performs all assessments in an accurate and timely manner.
  • Participates in the on-site 6 month review with the ALM and as needed as concerns arise.
  • Recommends changes in level of care within the facility to meet the needs of residents as needed.
  • Manages the clinical oversight of resident care and consults with residents, families, physicians and pharmacy personnel as needed.
  • Implements resident's plan of care as well as related daily/weekly schedule based on lifestyle preference needs and physical assessment and attends Service Plan meetings.
  • Abides by the Neighborhood Approach to caring for people living with a memory challenge or diagnosis, including assisting during meal times and ensuring dietary needs are met.
  • Conducts and documents a thorough assessment of each resident's medical status upon admission and throughout the resident's course of treatment.
  • Provides safe, direct physical care of the resident.
  • Maintains a clean, safe and orderly resident environment by keeping the area clean and disposing of soiled, contaminated items.
  • Reports malfunctioning, broken, or hazardous equipment or conditions to the supervisor.
  • Observes policies and procedures as demonstrated through performance of procedure and tasks.
  • Completes required documentation in an accurate and timely manner.
  • Oversees care delivery for the shift and receives/shares pertinent information with the prior and oncoming shift.
  • Other duties as assigned.
