About The Position

The Dedicated Recovery Coach carries a high acuity caseload and works toward assuring smooth and effective delivery of services to all families in need. The Dedicated Recovery Coach serves as a participating and facilitating member of the client’s team by providing services individually or as part of a team.

Requirements

  • Master’s Degree in Social Services or related field with or without AZ state licensing
  • Ability to obtain and maintain a valid unrestricted Level One Fingerprint Clearance Card.
  • Minimum age requirement is 21.
  • Must meet the requirements of a safe driver.
  • Must possess a current Arizona State Driver’s License and have an acceptable driving record for the past three (5) years.
  • Must have personal vehicle liability insurance and vehicle registration as required.
  • When required, the ability to pass a criminal background clearance check, drug screen, and if requested a physical exam.
  • Must obtain and maintain CPR and First Aid certification upon hire.
  • Strong knowledge of the behavioral health systems processes, practice, and components.
  • Skills, knowledge, and ability to instruct clients in the following topics: child development, home management, personal problem solving, accessing community services, and supports domestic violence issues, appropriate discipline and childcare techniques and age-appropriate parenting skills.
  • Ability to respond effectively displaying professionalism, sensitivity, and empathy to linguistic, cultural, social, economic, and individual differences and challenges. Ability to write and maintain accurate and timely documentation. Ability to present information 1:1 and to groups in an organized and concise manner.
  • Must be able to work independently as well as a member of a team. Must have excellent organizational and time management skills. Must remain flexible and able to handle multiple priorities with the ability to adjust to high pressure and rapidly changing business conditions.

Nice To Haves

  • 2 years’ experience in social work or related field preferred.
  • Bilingual in Spanish preferred

Responsibilities

  • Develop and implement individual service plans (ISP) in collaboration with the family and clinical team.
  • Assist clients in navigating the behavioral health system and make community referral as necessary.
  • Responsible for service coordination by assisting clients in implementing their plan of care by facilitating access to and providing community resource linkage and managing the process to include monitoring effectiveness of support services including both client and provider participation
  • Provide professional support to clients by maintaining regular contact to provide personal support, consultation, confrontation, and intervention, as necessary, including guidance and instruction.
  • Work collaboratively with family members, therapists, and other members of the care team to ensure comprehensive support.
  • Complete comprehensive evaluation of each client’s life situation and provide appropriate assessment and crisis planning as needed to address each client’s individual needs.
  • Gather information from community resources which may include schools, PCP, or other necessary resources.
  • Ensures that all services, procedures, and forms provided are culturally relevant, linguistically appropriate and gender responsive (including Lesbian, Gay, Bisexual, Transgender, and Questioning (LGBTQ).
  • Ensures that all clients are seen according to contract requirements.
  • Maintains accurate and confidential records of client interactions, progress, and outcomes and documents all records in the Electronic Health Record (EHR).
  • Completes the required number of job-related trainings according to all state and contractual required guidelines.
  • Ensures the safety, health, and welfare of staff and clients at all times.
  • Acts as a positive role model and mentor for children, parents, and their families.
  • Treats others with respect, confronts negative behavior, and supports confrontation.
  • Attends and participates in all required meetings including individual supervision and team meetings.
