The Montana Department of Public Health and Human Services (DPHHS) has a career opportunity within the Behavioral Health & Developmental Disabilities Division. The Developmental Disabilities Program procures and supports community-based services for people with developmental disabilities and/or their families. The DDP Targeted Case Manager is responsible for a comprehensive assessment and periodic reassessment of individual needs, the development of and periodic revision of the plan of care, referral and related activities to help the member obtain needed services, and monitoring and follow-up activities. The Case Manager is responsible to ensure the quality of the plan of care.
