The Registered Nurse (RN) will provide direct support to people with disabilities for routine health care, conduct various assessments, address minor illnesses/injuries and ensure proper follow-up. The RN will make referrals to other medical professionals/facilities as appropriate and serve as a member of the Interdisciplinary Team, participating in the ISP planning and implementation processes as required. The RN will ensure adequate nursing personnel are always scheduled for the homes they oversee, provide supervisory oversight to Trained Medication Employees (TME), and complete nursing personnel timekeeping/related functions in a timely fashion. Additionally, the RN will provide general program support, complete monthly nursing notes for the person served, and ensure upload in the appropriate mediums. The RN will conduct meetings with the Residential Coordinator (RC) and Qualified Intellectual Disability Professional (QIDP) and ensure Direct Support Professionals (DSP) are trained bi-annually and if there is any change in the person's health, RC, QIDP and DSPs are informed and trained.
