About The Position

The incumbent provides direct training and support services to individuals with intellectual disabilities receiving services from Crossroads CSB.  Responsibilities include developing and modifying individual person-centered service plans, documenting, and evaluating progress, supervising individuals, and providing training in personal care, home living, community living, pre-vocational, vocational, and behavior management skills.  This position floats between day support program locations as needed. The incumbent reports to and receives supervision from the site director/manager.  OSHA regulations apply to this position.  The position is designated as Level Two with regard to the HIPAA Privacy Rule.   The position is non-exempt for FLSA.

Requirements

  • Knowledge of intellectual disabilities.
  • Knowledge of training techniques for working with individuals with intellectual disabilities.
  • Knowledge of positive behavioral support techniques.
  • Knowledge of the individual service planning process.
  • Knowledge of non-aversive techniques for intervening with individuals who exhibit inappropriate behavior.
  • Knowledge of safe lifting and transferring techniques.
  • Knowledge of applicable Human Rights regulations and Crossroads CSB Human Rights policy.
  • Knowledge of The Health Insurance Portability and Accountability Act of 1996 (HIPAA) and the Department of Health and Human Services (DHHS) security and privacy regulations as they relate to the protection of confidential medical information and personal health information as required by law, professional ethics, and accreditation requirements.

Responsibilities

  • Implements skill training and behavioral support with individuals in accordance with person centered plans.
  • Supports individuals with attending to personal care needs, including providing physical assistance to individuals who require assistance with ambulation.
  • Records accurate progress data and completes written progress reports.
  • Evaluates individual progress and modifies person centered plans when progress review indicates need for changes.
  • Assists individuals with accessing community services and activities.
  • Provides transportation services to individuals.
  • Performs assessments of skill levels and support services needs utilizing various assessment formats approved by program director/manager.
  • Works with program director/manager and service planning team to develop person centered plans for assigned individuals, including development of support programs and assisting program director/manager with development of behavior management plans.
  • Maintains records for assigned individuals in accordance with licensure, Medicaid, Department of Labor, and other applicable regulatory and funding agency guidelines.
  • Intervenes effectively using non-aversive techniques with individuals who exhibit inappropriate behavior.
  • Support individuals with self-administering medication.
  • Responds to medical, behavioral, and other emergencies appropriately and in accordance with program policies and procedures.
  • Works cooperatively with other service providers to ensure service coordination for assigned individuals.
  • Follows appropriate safety procedures to prevent accidents/injury, to provide a safe and comfortable environment for individuals and to ensure compliance with OSHA guidelines.
  • Communicates oral and written information accurately and promptly to co-workers and supervisor.
  • Schedules activities and/or procures contract work for individuals as assigned.
  • Performs routine household maintenance and clerical tasks.
  • Participates in staff meetings and staff development activities.
  • Responsible for other duties as assigned consistent with job description/expectations and with program mission.
