Responsible for establishing, organizing, coordinating and supervising the Day Service Program including individual Group Leaders, work sites, community activities, on site as well as offsite activities. Day Service programs are to provide person centered valued activities, community inclusion and community integration. Must have an understanding of the purpose of the Day Service Center to provide a learning environment that focuses on individual strengths and abilities. Must have an understanding of the consumer's Person Center Plan, Support Profile and Behavior Management Plan. Ability to supervise and assist staff as well as the person served in their preferred activity setting(s). The Day Service Coordinator will provide a safe and therapeutic learning environment for the consumer in their preferred work/activity setting. The Day Service Coordinator will supervise and direct activities in various assigned work/activity areas and be responsible for hands-on training and support of the consumer as well as the Group Leaders and support staff assigned to the Day Service area(S). Day Service Coordinator will direct and monitor that work/activities are provided in a safe environment which is the least restrictive possible, while fostering a culture of consumer driven choice and community inclusion.

Requirements

  • High school graduate or G.E.D. certificate.
  • Preferred college degree in a related field (health care/social/management).
  • At least two years of experience in providing services to the intellectually disabled and/or developmentally disabled.

Responsibilities

  • Establish, organize, coordinate and supervise the Day Service Program.
  • Oversee individual Group Leaders and work sites.
  • Coordinate community activities both on-site and off-site.
  • Provide person-centered valued activities and promote community inclusion.
  • Understand and implement the consumer's Person Center Plan, Support Profile, and Behavior Management Plan.
  • Supervise and assist staff and consumers in their preferred activity settings.
  • Provide a safe and therapeutic learning environment for consumers.
  • Direct and monitor activities in assigned work/activity areas.
  • Provide hands-on training and support to consumers and staff.
