Requirements
- This role requires a highly professional, personable individual who can thrive both independently and as part of a team.
- The Home Health Aide will gain valuable experience in patient care, with opportunities for future growth and advancement based on performance as well as successful completion of additional training/certifications.
Responsibilities
- Providing direct, individualized care in a structured day program environment
- Assisting with personal care tasks such as bathing, grooming, dressing, and oral hygiene
- Supporting therapeutic exercises and activities as guided by on-site physical, occupational, and speech therapists
- Administering medications under the supervision of an on-site Registered Nurse (RN)
- Observing and reporting changes in patients’ conditions to RNs or therapists
- Promoting a safe and supportive environment, reporting any potential safety concerns
- Accurately documenting patient care and activity as required
- Performing additional duties based on individual care plans or team direction
