About The Position

Responsibilities

  • Provide direct support, supervision, and training for individuals with disabilities.
  • Assist with feeding, bathing, dressing, and mobility of residents.
  • Maintain and enter daily records in the Therap system.
  • Provide transportation and supervision for individuals to meet employment and recreational activities.
  • Perform inspections of vans and ensure safe transport training is followed.
  • Conduct housekeeping responsibilities in various areas of the facility.
  • Prepare meals according to individual healthcare plans and manage diets.
  • Complete documentation and notes as required using the Therap system.
  • Conduct evacuation drills and record results.
  • Assist in planning and facilitating person-centered outings.
  • Complete behavioral tracking logs as required.
  • Follow all agency, state, and federal laws and policies.
  • Manage medical emergencies according to health service plans.
  • Attend and transport residents to medical appointments as required.
  • Identify and report maintenance issues to the supervisor.
  • Administer medications and track vital signs for residents.
