About The Position

ACHIEVE is seeking caring staff members for immediate human services openings to positively impact the people they serve. The Day Habilitation Program focuses on promoting independence and support for individuals with moderate to severe disabilities in the Human Services field. This role involves activities both inside facilities and in the community, such as enjoying local parks, shopping, eating at local restaurants, and volunteering for local organizations. The position is full-time with paid training provided, and no prior experience is necessary.

Requirements

  • High School Diploma or GED
  • Possess a valid driver’s license acceptable to the agency’s insurance carrier
  • Covid Vaccine -OR- an approved medical or religious exemption

Responsibilities

  • Implement goal plans and data collection to monitor progress.
  • Transport individuals to volunteer and recreational activities in the Community.
  • Provide the necessary supervision, training and support during community outings.
  • Maintain accurate daily participant records, charts, logs, and case/progress notes per the instruction of the Direct Supervisor and the individual treatment plan methodologies.
  • Maintain an orderly, clean, safe, environment at all times.
  • Order supplies with approval from the Direct Supervisor.
  • Assist an individual with toileting, if needed.
  • Complete monthly progress notes, semi-annual and annual reviews as assigned by or in the absence of the Direct Supervisor.
  • Assist in boarding vehicles and disembarking from vehicles.
  • Monitor passengers and maintain orderliness on vehicle to ensure passenger safety and adherence to safety regulations.
  • Assist in planning activities which will foster community integration.
