This LPN position will be held in a Day Habilitation setting. The LPN would be providing basic care to a large group of individuals, and would have the same individuals on each shift. Requirements

  • Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing.
  • At least one year of full time working experience as an LPN.
  • Must be 21+ years old, have a valid driver's license, daily use of a car on site and be available by telephone for daily contact.

Nice To Haves

  • Willingness to support other day programs based on program needs, with travel up to 50 miles per day.

Responsibilities

  • Assessments specific to each person's health care needs.
  • Administration of medications as assigned.
  • Write progress notes and weekly nursing assessments.
  • Notify the Health Care Provider of any medical changes or concerns then communicate and document all information.
  • Communicate any discharge instructions, treatments or medication changes to all staff.
  • Physically assist individuals in transfer and carries, both ambulatory and non-ambulatory.
  • Physically assist persons in personal hygiene tasks which may include lifts and physical support to persons with body weights of 150 - 250 pounds.
  • Educate and assist staff and individuals with nutrition and dietary needs.
  • Follow agency policies and procedures as defined in the agency's Policy and Procedure Manual.
  • Identify safety risks and possible alternatives and advocate for the rights of all individuals.
  • Maintain confidentiality in keeping with HIPPA regulations.
  • Handle related job duties in compliance with agency's Programmatic Policy Manual and Medicaid regulations.
  • Responsible for implementing ROM routines and documenting in accordance with the plans.
