Community Connections is a Day Habilitation program that serves adults between the ages of 23-66 with a primary diagnosis of intellectual disabilities, though some individuals may also have mental health diagnoses and/or physical challenges. Most participants require supportive structures and staff assistance to maximize their involvement in activities within the program as well as within the local community. Our staff educate through the creation and implementation of individualized service plans, daily curriculum/activities, and have a heavy focus on community integration. The Day Hab Director/Service Manager is responsible for the oversight, day-to-day activities, and delivery of services at the Community Connections Day Hab Program. They provide leadership, supervision, and support to the team of Developmental Specialists and Teaching Assistants, ensuring they are implementing service plans, meeting Mass Health regulations and all funder requirements of the program. The Day Hab Director/Service Manager also collaborates with clinical staff, the nursing department and external consultants to ensure individuals receive optimal support, are educated about their rights, and are provided advocacy on their behalf, when needed.
