About The Position

Community Living Corporation is seeking a Day Hab Instructor to join our team! Day Hab Coordinator - Full-Time Full-Time Employee Health Insurance Paid in Full The Day Habilitation Coordinator is responsible for facilitating the community outings, design and implement daily activity schedules and provide consistent structure and routine in a creative environment. The position requires a detail-oriented, organized and outgoing individual to serve in the Without Walls program. This is a community-based role responsible for the development and leading therapeutic and creative programming for adults with developmental disabilities. This position is for an enthusiastic and compassionate while promoting independence, social inclusion, personal growth through meaningful activities in diverse community settings leading the Westchester County based program.

Requirements

Must possess a HS Diploma/GED, valid driver's license for a minimum of 3 years, and own transportation.

Responsibilities

Facilitate Day Habilitation paperwork, including: Review and maintain client files to ensure all required documentation is intact and up to date
Review program service documentation to ensure data has been recorded accurately and completely
Complete biweekly client timesheets and compare service documentation with timesheets to ensure accuracy
Work closely with Medicaid Billing Coordinator to ensure service documentation is intact/complete for all claims billed
Assist Program Director with organization/preparation of daily/biweekly/monthly files
Assist with program enrollments for new clients
Track all renewal dates for clients’ program plans to ensure plans are kept current
Follow up with Program Coordinator regarding any noncompliant items
Organize and Implement program at main office location
Implement daily, weekly, monthly schedule of activities
Coordinate activity transportation ensuring all participants are scheduled
Assign appropriate staff and participants for daily scheduled activities
Monitor client engagement to ensure positive program outcomes
Support individuals in the creative of client focused programming
Provide “Active Treatment.” “Active treatment” is defined as the application of therapeutic strategies taught in PBS and clinical trainings under the guidelines of the Behavioral Mental Health Model. Utilize philosophy of the therapeutic model and apply this knowledge to daily work.
Assist in inspections of vehicles
Supervise all clients in group
Assist clients requiring additional help
Provide instruction on proper work techniques
Set positive example by working alongside clients
Provide positive reinforcement as needed
Maintain daily log book; describing the day’s activities, location and clients present.
Report any problems/concerns to the Director upon return to office.
Ability to collaborate and communicate effectively with all staff members including Administration, Accounting, Residential, Health Services, Human Resources, Purchasing, Maintenance and Day Hab.
Assist coworkers with miscellaneous projects and duties as assigned
Perform other tasks as requested by the Day Habilitation Director or Executive Director
