About The Position

Are you a compassionate caregiver with a heart for helping others? As a Participant Care Assistant (PCA) in our day center, you'll play a vital role in ensuring our participants receive the highest quality care. From assisting with daily activities to providing comfort and companionship, your work will make a meaningful impact in the lives of those we serve.

Requirements

  • Certified Nursing Assistant Certification in the state of MI, required.
  • Valid Michigan Driver’s License with a clean driving record
  • Current CPR or BLS certification or willing to obtain.
  • Ability to work independently, multitask, and operate as part of an interdisciplinary team
  • Ability to lift and move objects weighing up to 50 pounds without assistance.

Nice To Haves

  • One (1) year of prior experience with frail or elderly individuals, aiding with daily living activities, and offering compassionate care, is highly desired.

Responsibilities

  • Communicates effectively with participants, families, staff and the staff of other agencies
  • Participates in Interdisciplinary Team (IDT) meetings as directed
  • Provides basic ADL nursing care to participants
  • Documents observations, nursing care and participant activities on appropriate forms and in the medical record as appropriate
  • Reports changes in the participant’s condition to the pertinent IDT member
  • Active participation in the Quality Improvement Process (QIP)
  • Clean facility equipment and maintain clean facility condition
  • Reports on participant status or family concerns to the DCS
  • Completes home visits when directed
  • Abides by all established PACE of Southwest Michigan policies, rules and regulations
