About The Position

Under the supervision of the Center Director, manages the daily operations of the St. Paul’s PACE Program Day Center. Responsibilities include the planning, coordination and implementation of all activities in the Day Center as well as the supervision of the Day Center Workers.

Requirements

  • High School Diploma or equivalent required.
  • Shall be at least 18 years of age.
  • A Minimum of one year’s experience in working with the frail or elderly population required.
  • One year of supervisory experience, preferably in a geriatric care setting required.
  • Shall have either training or related experience in the job assigned.
  • Knowledge of principles, practices standards and techniques of a CNA
  • Working knowledge of the PACE regulations.
  • Working knowledge of San Diego’s healthcare and aging networks.
  • Demonstrated effective leadership and decision-making skills.
  • Effective supervisory skills and ability to foster collaborative working relationships.

Responsibilities

  • Directs, supervises, and ensures completion of initial and annual competencies on, and evaluates the performance of the Day Center employees.
  • Provides/coordinates in-services, and training for Day Center employees according to program needs, and state and federal regulatory requirements.
  • Ensures that the job responsibilities, authorities and accountability of all direct reports are defined and understood.
  • In conjunction with the Center Director is responsible for maintaining compliance with all state and federal program requirements, obtaining and maintain certification under CMS as well as any other state or federal law or rules
  • Functions as a member of the interdisciplinary team.
  • Maintains regular attendance at, and participants in interdisciplinary team meetings; communicates participant changes, collaborates on care planning decisions and coordination for 24 hour care delivery.
  • Provide leadership on the Interdisciplinary Team, through participation in development of the Plan of Care and monitoring and articulating participant’s progress and decline.
  • Maintains the day center and its equipment as required by plant operations, and state and federal agencies.
  • Monitors operations to ensure consistent compliance with established quality assurance standards.
  • Recommends changes in service delivery and staffing as required by the development and growth of the agency.
  • Participates regularly in all quality assurance/quality improvement committees as part of the overall quality improvement program.
  • In conjunction with the Center Director and Quality Assurance team, maintains the stability and reputation of St. Paul’s PACE by insuring that all activities and operations are performed in compliance with local, state, and federal contracts, regulations, laws, accreditation, protocol, licensing and certification requirements governing St. Paul’s PACE operations.
  • Fulfills all administrative requirements, such as development of Day Center Workers schedule, over sight and completion of daily-required documentation and reports associated with St. Paul’s PACE, State and Federal agencies.
  • Practices Universal Precautions and follows appropriate Infection Control procedures.
  • Monitors St. Paul’s PACE operational compliance with OSHA regulations and agency policies and procedures.
  • Protects privacy and maintains confidentiality of all company procedures, results, and information about, employees, participants, clients or families.
  • Participates in continuing education classes and any required staff and training meetings.
  • Maintains professional affiliations and any required certifications.
  • Performs related duties as required or requested.
