Requirements

  • Master's or Doctorate degree in Nursing from an accredited program.
  • Current and unrestricted Nurse Practitioner license in the state of Maine.
  • Certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner.
  • Minimum of 2 years of experience as a Nurse Practitioner, preferably in long-term care settings.
  • Strong interpersonal and communication skills with the ability to collaborate effectively with diverse healthcare professionals.
  • Dedication to providing compassionate and patient-centered care.
  • Knowledge of electronic health records (Epic) and proficiency in relevant medical software.

Responsibilities

  • Conduct comprehensive assessments of patients in long-term care facilities, including medical history, physical examinations, and medication reviews.
  • Develop and implement individualized care plans to address the unique needs of long-term care residents.
  • Collaborate with interdisciplinary healthcare teams to coordinate and optimize patient care.
  • Provide education and support to patients, families, and caregivers on disease management, preventive care, and overall health and wellness.
  • Monitor and manage chronic conditions, collaborating with physicians and specialists as needed.
  • Participate in regular team meetings and contribute to the development and improvement of patient care processes.
  • Stay abreast of current healthcare trends, regulations, and evidence-based practices related to long-term care.
